How to enroll yourself :
1) Fill the application form after studying the conditions.
2) Enclose it with a cheque / cash of Rs. 100/- in favour of our company and handover the same to your sponsor, or to the nearest Medical Shop as per the list enclosed or to our office.
3) You will be given on I.D. to enable you to enroll further members in your account. Please affix a stamp size photograph of yours on the I.D. Card, carry it always with you.
